Transcend helps prepare people leaving incarceration for long-term recovery in the community.
Transcend is a reentry service for people who are returning to our community from incarceration. Transcend offers pre- and post-release recovery support and wrap-around services. We support people in recovery as they plan for life in our community.
Our goal is to help participants achieve long-term recovery and independence.
Our CasaCare model weaves together SUD/COD treatment, medical care, and support services. We also connect patients with community-based resources and support.
Transcend patients may enroll in our co-located wellness services.
Transcend serves individuals who:
- Adults (18+)
- Preparing for reentry from corrections
- Have been Justice Involved at some point in their lives
- Express interest in addressing their substance use and working towards their recovery
Thanks to funding support from BSAS, Casa is able to provide priority placement for Black and Latino adult men in Suffolk County Jail.
Transcend Services:
- Pre- and post-release case management/care coordination for your whole family
- Recovery support services
- A personal care plan with short- and long-term wellness goals
- Referrals to support services and community resources
- Support reconnecting with the people who are important to you
- Connections to reintegration services, including jobs fairs, skills-building courses, and more!
