(CSP)

Community Support helps participants assess their needs and achieve recovery goals.

The Community Support Program (CSP) provides an array of services delivered by community-based, mobile care coordinators who are supported by a clinical supervisor to patient with psychiatric or substance use disorder diagnoses and/or to patients for who their psychiatric or substance use disorder diagnoses interfere with their ability to access essential medical services.

These programs provide support services that are necessary to ensure members access and utilize behavioral health services. CSP programs do not
provide clinical treatment services, but rather provide outreach and support services to enable patients to utilize clinical treatment services and other supports.

Our program tailors services to the unique needs of each patient. All treatment is free of charge.

CSP serves adults (18+):

  • With psychiatric and substance use disorder(s) (SUDs) or
  • Co-occurring (CODs) SUD(s)

CSP Services:
  • Assist clients in improving their daily living skills so that they can perform them independently
  • Provide service coordination and linkage to community resources
  • Provide temporary assistance with transportation to essential medical and behavioral health appointments
  • Assist with obtaining benefits, housing, and healthcare
